I usually use about a 24" leader and haven't had any trouble with the surgeons going through the guides. 

We even started using the surgeons knot when tying the backing to the power pro.  This does take an extra hand to pass the spool through the loop, but it is still quicker than the Uni to Uni.

I haven't had any problems with it hanging up in the guides.  It's a small knot if you cinch it down good and trim the tags as already stated.
